Please read all the instructions below.
  1. Obtain the Lima Crossroads Room Reservation Form (Pink).
  2. Complete the form in its entirety. (It's better to have too much information). We especially need your phone number so we can confirm your reservation or obtain futher information.
  3. Sign the form.
  4. Return the completed form to the church office.
  5. Church ministries will be given priority over outside organizations or events
  6. As long as we have the required information, we will respond to your request within one or two working days.
  7. If you have any questions or concerns, contact the church office.
